This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please read it carefully.

  

 
 


Seven Oaks OMFS is dedicated to protecting your privacy and the security of all Protected Health Information as described by federal law. At times it may be deemed necessary for us to share your information with other Health Care Providers or Business Associates. The following situations are examples of instances where information may be shared:

    
 
  • Treatment regarding laboratory analysis.
  • Payment from you, your insurance companies or a third party.
  • For healthcare operations regarding appointment reminders or communication with persons involved with decision making regarding your treatment.
  • For legal purposes as required by federal, state, or local law; or in the case of lawsuits or other legal disputes; or if ordered to do so by any law enforcement agencies.
  • Workers compensation or similar financial reimbursement programs requesting information regarding your treatment circumstances.
  • If coroners, medical examiners or funeral directors request X-Rays or medical records to identify a deceased person or determine the cause of death.
  We here at Seven Oaks OMFS are committed to obeying all Federal, State and Local laws and regulations regarding Privacy Practices. In the event that there are any necessitating circumstances that require us to disclose information about you, other than the ones listed above, we will only release that information with the written authorization of the individual involved.
